Additional Practice Details
Serve individuals 14 years and older; all Medicaid forms accepted in Clermont and Hamilton; Additional insurances accepted: Value Option, United Medical Resources, Tri-Care, and sliding scale options available for non-insured clients; Providers at this location: Sarah Porter, Kerry-Lyn Evans, Kelle Greeson, Jennifer Taylor, Jessie Baublitz, Jane Cohen, and Derek Stegner. Stegner and Cohen are the only providers who will work with children 12 years and up. The others will work with families.
During mandated social distancing due to COVID-19 all services will be held via video conferencing. There will be no face to face services or walk in hours at this time.
